RESUMO

BACKGROUND: Exercise therapy is an effective component of fibromyalgia (FM) treatment. However, it is important to know the effects and specificities of the different types of exercise: muscle stretching and resistance training. AIM: To verify and compare the effectiveness of muscle stretching exercise and resistance training for symptoms and quality of life in FM patients. DESIGN: Randomized controlled trial. SETTING: Physical therapy service, FM outpatient clinic. POPULATION: Forty-four women with FM (79 screened). METHODS: Patients were randomly allocated into a stretching group (N.=14), resistance group (N.=16), and control group (N.=14). Pain was assessed using the visual analog scale, pain threshold using a Fischer dolorimeter, FM symptoms using the Fibromyalgia Impact Questionnaire (FIQ), and quality of life using the Medical Outcomes Study 36-item Short-Form Health Survey (SF-36). The three intervention groups continued with usual medical treatment. In addition, the stretching and resistance groups performed two different exercise programs twice a week for 12 weeks. RESULTS: After treatment, the stretching group showed the highest SF-36 physical functioning score (P=0.01) and the lowest bodily pain score (P=0.01). The resistance group had the lowest FIQ depression score (P=0.02). The control group had the highest score for FIQ morning tiredness and stiffness, and the lowest score for SF-36 vitality. In clinical analyses, the stretching group had significant improvement in quality of life for all SF-36 domains, and the resistance group had significant improvement in FM symptoms and in quality of life for SF-36 domains of physical functioning, vitality, social function, emotional role, and mental health. CONCLUSIONS: Muscle stretching exercise was the most effective modality in improving quality of life, especially with regard to physical functioning and pain, and resistance training was the most effective modality in reducing depression. CLINICAL REHABILITATION IMPACT: The trial included a control group and two intervention groups, both of which received exercise programs created specifically for patients with FM. In clinical practice, we suggest including both modalities in an exercise therapy program for FM.

OBJECTIVES: Considering the subjectivity of quality of life measurements, the aim of this study was to verify the discriminating power of two quality of life questionnaires in patients with fibromyalgia: the generic Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36) and the specific Fibromyalgia Impact Questionnaire (FIQ). METHODS: A cross-sectional study was conducted on 150 participants divided into Fibromyalgia Group (FG) and Control Group (CG) (n=75 in each group). The participants were evaluated using the SF-36 and the FIQ. The data were analyzed by the Student t-test (α=0.05) and inferential analysis using the Receiver Operating Characteristics (ROC) Curve--sensitivity, specificity and area under the curve (AUC). The significance level was 0.05. RESULTS: The sample was similar for age (CG: 47.8 ± 8.1; FG: 47.0 ± 7.7 years). A significant difference was observed in quality of life assessment in all aspects of both questionnaires (p<0.05). Higher sensibility, specificity and AUC were obtained by the FIQ (96%, 96%, 0.985, respectively), followed by the SF-36 (88%, 89% and 0.948 AUC). CONCLUSION: The FIQ presented the highest sensibility, specificity and AUC showing the most discriminating power. However the SF-36 is also a good instrument to assess quality of life in fibromyalgia patients, and we suggest that both should be used in parallel because they evaluate relevant and complementary aspects of quality of life.

CONTEXTUALIZAÇÃO: A fibromialgia é uma síndrome dolorosa caracterizada por dor espalhada e crônica e sintomas associados com um impacto negativo na qualidade de vida. OBJETIVOS: Considerando a subjetividade da mensuração de qualidade de vida, o objetivo deste estudo foi avaliar o poder de discriminação de dois questionários que avaliam a qualidade de vida de pacientes com fibromialgia: o genérico Medical Short Form Healthy Survey (SF-36) e o específico Questionário do Impacto da Fibromialgia (QIF). MÉTODOS: Foi conduzido um estudo transversal com 150 indivíduos, divididos em dois grupos: grupo fibromialgia (FM) e grupo controle (GC) (n=75 em ambos). Os pacientes foram avaliados pelo SF-36 e pelo QIF. Na análise dos dados, utilizou-se o teste "t de Student" com α=0,05 e a Curva ROC (Receiver Operating Characteristics Curve). RESULTADOS: As amostras foram estatisticamente semelhantes para a idade - 47,8 (8,1) no GC e 47,0 (7,7) no FM - e estatisticamente diferentes em todos os aspectos dos dois questionários (SF-36 e QIF). Alta sensibilidade, especificidade e área abaixo da curva (AUC) foram obtidas com o QIF (96 por cento, 96 por cento, 0,985 respectivamente), seguido pelo SF-36 (88 por cento, 89 por cento e 0,948 AUC). CONCLUSÃO: O QIF mostrou-se mais discriminativo do que o SF-36 para avaliar a qualidade de vida de fibromiálgicos. No entanto, o SF-36 é também um bom instrumento de avaliação e sugere-se que ambos sejam usados uma vez que avaliam aspectos relevantes e complementares da qualidade de vida.

OBJECTIVE: The aim of this study was to verify the discriminative power of the most widely used pain assessment instruments. METHODS: The sample consisted of 279 subjects divided into Fibromyalgia Group FM- 205 patients with fibromyalgia and Control Group CG-74 healthy subjects), mean age 49.29 +/- 10.76 years. Only 9 subjects were male, 6 in FM and 3 in CG. FM were outpatients from the Rheumatology Clinic of the University of São Paulo--Hospital das Clínicas (HCFMUSP); the CG included people accompanying patients and hospital staff with similar socio-demographic characteristics. Three instruments were used to assess pain: the McGill Pain Questionnaire MPQ, the Visual Analog Scale (VAS), and the Dolorimetry, to measure pain threshold on tender points (generating the TP index). In order to assess the discriminative power of the instruments the measurements obtained were submitted to descriptive analysis and inferential analysis using ROC Curve-sensibility (S), specificity (S1) and area under the curve (AUC)--and Contingence tables with Chi-square Test and odds ratio. Significance level was 0.05. RESULTS: Higher sensibility specificity and area under the curve was obtained by VAS (80% 80% and 0.864, respectively), followed by Dolorimetry (S 77% S177% and AUC 0.851), McGill Sensory (S 72% S167% and AUC 0.765) and McGill Affective (S 69% S1 67% and AUC 0.753). CONCLUSIONS: VAS presented the higher sensibility, specificity and AUC, showing the greatest discriminative power among the instruments. However, these values are considerably similar to those of Dolorimetry.
